1. Assumption of Risk
I understand that participating in a fitness training program involves physical activity that may include, but is not limited to:
• strength training
• cardiovascular exercise
• flexibility training
• mobility exercises
• use of gym equipment
• bodyweight movements
I acknowledge that physical exercise carries inherent risks, including but not limited to:
• muscle soreness
• muscle strains or tears
• joint injury
• heart complications
• dizziness or fainting
• other physical injury
I voluntarily assume all risks associated with participation in this program.
2. Medical Clearance
I confirm that:
• I am in good physical condition to participate in exercise
• I do not have any medical conditions that would prevent safe participation, OR I have informed my coach of any relevant conditions
• I have consulted with a physician if I have concerns about beginning an exercise program
• I will inform my coach immediately if I experience pain, discomfort, dizziness, or other symptoms during training
3. Release of Liability
I hereby release and discharge Nicolas Edwards from any and all liability, claims, demands, or causes of action that may arise from participation in any fitness program, coaching service, or related activities.
This includes, but is not limited to:
• injuries sustained during exercise
• injuries resulting from use of recommended equipment
• injuries resulting from following programming or instruction
• injuries occurring during in-person or online coaching
I understand that I am participating voluntarily and accept full responsibility for my participation.
4. Online Coaching Disclaimer
I understand that online coaching involves performing exercises without the coach physically present.
I acknowledge that:
• I am responsible for using proper form and technique
• I am responsible for choosing appropriate weights and intensity levels
• I will stop any exercise that causes pain or discomfort
• I may submit videos for form feedback, but real-time supervision is not guaranteed
5. Nutrition Guidance Disclaimer
I understand that any nutrition guidance provided is for educational purposes only and is not medical or dietetic advice.
I acknowledge that:
• the coach is not acting as a physician or registered dietitian
• I am responsible for my own food choices
• I will consult a qualified medical professional if needed
6. Client Responsibility
I agree to:
• follow instructions to the best of my ability
• ask questions when unsure about exercise technique
• use equipment safely
• communicate any injuries or limitations
• take responsibility for my own health and wellbeing
7. Acknowledgment of Understanding
I have read this document fully and understand its contents.
I acknowledge that:
• I am voluntarily participating in fitness coaching
• I understand the risks involved
• I agree to release the coach from liability as described above
